RESUMO

The collaborative project between the University of Las Palmas de Gran Canaria and the public University of Mozambique, UniZambeze, aims to strengthen the institutional and training capacities of its Faculty of Medicine located in Tete to provide new medical graduates. The essence of the program, training doctors, has the objective of improving the healthcare system and making it sustainable for the integration of new graduates into the staff of the faculty. In this work, we determine the cost of education for a new doctor and we evaluate the social benefit of the program in terms of the human capital. The program has led to the training of 199 new doctors in the 11 years of operation and is leading the way for 100 more in the next four years. The incorporation of some of them into the faculty's staff will generate new doctor graduates in the near future with a cost below 6000 EUR each in normal circumstances. These results can help to determine how much traditional international aid healthcare programs can save when investing in the education of new doctors. This program is an alternative policy for the international aid financing budgets of donor countries. Supporting teachers and native doctors in the future with private and public patronage programs can raise the efficiency per EUR spent.

BACKGROUND: An isthmocele is a scar defect in the uterine wall due to a cesarean section. Its prevalence is unknown, but it has been associated with secondary infertility. Here, we present a case where the patient suffered from an isthmocele that was associated with persistent hydrometra, which developed during in vitro fertilization. CASE SUMMARY: The patient underwent hysteroscopic surgery, which successfully resolved the isthmocele as well as the hydrometra. Afterward, two high-quality, euploid embryos, determined by morphological assessment and pre-implantation genetic diagnostic testing, were transferred. This resulted in uterine pregnancy, as determined by serum ß-human chorionic gonadotropin levels on day 14 (180 mU/mL) and ultrasound-confirmed presence of a gestational sac with a positive embryocardia at week 6. The pregnancy reached 36 wk without any complications, and the product was born in good health. We report a successful isthmocele treatment in a patient with secondary infertility, in which the isthmocele was the cause of persistent hydrometra. CONCLUSION: Hydrometra caused by secondary cesarean is an infertility factor, which can be corrected by hysteroscopy plus ablation of the isthmocele.

Todos los países, cualquiera que sea su grado de desarrollo, se esfuerzan por proporcionar unos cuidados sanitarios adecuados a sus ciudadanos. Gozar de buena salud es una condición básica para el desarrollo y la seguridad de las personas, a pesar de lo cual persisten enormes desigualdades en salud entre los países. Los educadores médicos tienen un papel clave en la reducción de estas desigualdades al abordar uno de los aspectos esenciales del problema: la acusada escasez de médicos que sufren algunas regiones, especialmente en el África subsahariana. Una de las mayores dificultades para afrontar esta cuestión es la falta de profesores cualificados, lo que podría solventarse mediante estancias cortas de graduados procedentes de países desarrollados que voluntariamente quisieran participar en este tipo de iniciativas. Pero el mundo de la educación médica tampoco es un terreno justo, equilibrado y equitativo, y trabajar en países pobres y alejados supone un reto importante. En este artículo, presentamos la experiencia en el diseño y desarrollo de un proyecto de formación de médicos en la Universidad de Zambeze, en Tete (Mozambique), llevada a cabo desde la Universidad de Las Palmas de Gran Canaria (AU)


Every nation-ich or poor and developed or developing-strives hard to deliver optimal healthcare to its citizens. Good health is the necessary condition for development and human security, but inequities in health persist. Medical educators have a key role to play in reducing inequities in global health by addressing the underlying doctor shortages that have reached crisis levels in some regions. The region in which these health inequities and doctor deficiencies are starkest is sub-Saharan Africa. In poor countries, a major constraint to approach this issue is the scarcity of qualified teachers. One of the options is the short-term placement of graduates from rich countries seeking opportunities to contribute in other countries that are severely deficient in faculty. But the world of medical education is not a level playing field and working in developing countries, and in remote areas in particular, can be challenging. In this article we show a recent experience carried out at the University of Las Palmas de Gran Canaria designing and supporting a project on undergraduate medical education at the University of Zambeze, in Tete, Mozambique (AU)

RESUMO

El dermatofibroma es una lesión cutánea común en la práctica dermatológica. La manifestación típica es una pápula de crecimiento lento, firme y solitaria. Desde el punto de vista histopatológico, se define en ocasiones como una reacción del tejido conectivo dérmico y en otras como una proliferación neoplásica benigna. En 1990 se publicó por primera vez la existencia de metástasis pulmonares de este tumor. Desde entonces se han recogido en todo el mundo 12 casos de similares características. Presentamos el caso de una mujer joven con un dermatofibroma recidivante en el hombro y metástasis pulmonares bilaterales. Analizamos qué características histológicas del tumor podrían hacer sospechar un comportamiento posterior inusualmente agresivo(AU)
